I have been a consultant neonatologist (newborn care specialist) for 30 years, including more than 8 years here in the Cayman Islands, bringing true neonatal expertise, compassion and a very calm disposition to deliveries. I am a passionate advocate for parental wishes alongside newborn safety. I am qualified both as a neonatologist (specialist newborn, preterm, baby doctor) and a paediatrician. I trained in some of the foremost neonatal intensive care units in the United Kingdom, and have been Chief of Service or no less than two tertiary NICUs, including one famous for its work in pioneering both ventilation of newborn infants and in the use of therapeutic hypothermia for birth complications. I pride myself on practising up-to-date, evidence-based medicine, with an unwavering passion for excellence and safety.

Training & Experience as Paediatrician Cayman Islands

I am delighted to share more about my training, background a range of experiences as a Consultant Paediatrician, both in the United Kingdom and then latterly here in Cayman Islands.

Qualifications & Paediatric Training

I was awarded a Bachelor of Medicine and Surgery with Honours from Manchester University in United Kingdom in 1986 and then went on to obtain the following:

I went through a full UK medical training including both Medicine and Surgical training at Manchester Royal Infirmary and University Hospital of South Manchester, both tertiary hospitals, before commencing my paediatric training in Stoke on Trent. I then went on to undertake multiple placements in world renowned Birmingham Children’s Hospital, including paediatric cardiology training.

Some of my more complex paediatrics training was then undertaken at the World famous Great Ormond Street Children’s Hospital. After this, I moved to the Midlands, with rotations in Derbyshire Children’s Hospital and then into Nottingham City Hospital.

Finally, I undertook an academic role as Lecturer in Child Health at Queen’s Medical Centre in Nottingham, before become a Consultant back at Nottingham City Hospital.

So how many years paediatric training?
After my basic medical and surgical training, I spent 8 years undertaking paediatric training, including in newborn paediatrics and neonatal medicine. This is the period of hands on practical care under a structured training programme. I was fortunate to gain virtually my whole training in some of the UK’s most prestigious tartiary centres.
What aspects of paediatric care are covered?
A UK paediatric training is designed to be extremely comprehensive, covering the full spectrum of general paediatrics care, including routine, complex paediatrics and developmental follow up. During rotations, you get further experience by being part of the paediatric on-call rota, during which you gain exposure to a very wide variety of paediatric conditions. I have a real depth of training in paediatric respiratory medicine, thanks to my time in Great Ormond Street.
Do you have Paediatric Nutrition expertise?
I actually have a great deal. For many years I ran a specialist feeding clinic in UCLH. I am very familiar with feeding difficulties, advising on problems such as cow’s milk intolerance, as well as addressing paediatric diabetes and weight management problems.

Paediatric Experience

I moved to Grand Cayman in 2016, after already amassing 22 years experience as a consultant paediatrician (and neonatologist). I have now added a further 8 years experience here too with what has been lovely, thriving and busy childrens clinic, newborn and neonatal service.

My practice is somewhat unusual because of my formal training and accreditation in both paediatrics and neonatal medicine, as well as a career lifetime of experience in tertiary medicine, exposing me to far more complex paediatric problems. Consequently, as ‘Paediatrician Cayman’ and besides my deliveries, neonatal and hospital care, I have a very varied children’s population, including more than my fair share of more complex care.

Complex Paediatric Care

The more complex conditions I manage include respiratory disease (for which I have a special interest and training from Great Ormond Street, not to mention a heavy respiratory caseload for the whole of my career), failure to thrive, short stature, diabetes, duchennes muscular dystrophy, complex cardiac disease (for which I have a specific training too) and immune deficiency.

Neurodiversity & ADHD

I have an additional special interest in neurodiversity, ADHD and autistic spectrum disorders, seeing children from early in their lives through to 18 years, with challenges from the very minor to complex health and developmental problems. Over my career, I have amassed very significant experience in assessing for, addressing and managing referrals for development issues, including learning and behavioural issues, as they tend to be more prevalent in ex-preterm infants. I’ve now been doing that for more than 30 years.

One of my own children has ADHD and is on the autistic spectrum, providing a very unique insight, including just how tough it can be for parents too. I am a passionate advocate for ensuring children with ADHD and/or neurodiversity-related challenges really thrive.

Paediatric Immunology

I also have a particular expertise in paediatric and newborn immunological problems, spanning both allergic issues and intolerances,  to viral and bacterial infections. I am helping (in an advisory capacity) to develop better tests to ensure children do not have bacterial infections late diagnosed, given the severe consequences that can have.

You never stop learning or gaining experience. I estimate that I have conducted more than 15,000 well child checks, attended more than 500 deliveries, delivered 2,000+ days of neonatal care and so much more in the 8 years I have been Paediatrician Cayman! That means I am becoming a more experienced paediatrician every day!

So how many years experience?
In paediatrics, including in newborn paediatrics and neonatal medicine, I have 37 years experience. However, that includes my training roles too and so a better question when asking a paediatrician, is how many years as a fully qualified consultant. I became a consultant in 1994 and so the answer to that is 30 years!
What sort of complex cases do you see?
Across 30 years as a consultant, especially with so many at tertiary level, I have treated a huge number of complex cases, from supporting children with severe developmental issues following prematuyrity or borth events, to very rare paediatric conditions or genetic disroders, such as Duchennes Muscular Dystrophy or complex paediatric cardiology.

Frequently Asked Questions

Here are some of the more common questions I get asked about my paediatric care and services.
Just how available are you?
If you asked my parents they’d say “Dr Sara is always available when we need her” and that comes from the fact that if I am here (and I travel very little), I am almost always available by phone or WhatsApp and usually within minutes unless I am already seeing a patient.
Do you see hotel & condo guests?
I do. I prefer to see them at my clinic (which is typically 10 minutes from most of Seven Mile) where I have all of my facilites and equipment. However, where medically prudent, I can see children at hotels or condos too. In the first instance please call me on +1 345 326 7827 or in clinic hours on +1 345 745 7070
What are your charges for hotel and condo guests?
I charge the exact same as I would for one of my registered patients permanently here. It’s based on somethign called the Standard Health Insurance Fees schedule. I do not charge higher rates just because someone is visiting.
Are you more expensive given your experience?
No. I charge what we refer to as Cayman Standard Fees based on the Standard Health Insurance Fees (SHIF) schedule. However, my approach to care is to not over-investigate e.g. by ordering 5 tests when 2 will give the answer, nor over treat. Parents frequently find themselves surprised at how low my overall charges are when compared to their experiences elsewhere. Some of this is a benefit of experience too, of course.
Do you charge for vaccines & immunisations?
Well, the first thing I always tell my parents is that they can always go to HSA for free vaccine services according to the Cayman Islands Immunisation Schedule. However, I also administer a great many vaccines as it allows parents to schedule and avoid waits. I make a small standard charge for the actual giving of the vaccine. Where I am giving vaccines that I can obtain from HSA, I make no charge for those. The only ones where I am charging are where we have to purchase them.

Can you help with behavioural, learning and ADHD issues?
I have a very large practice in this area and I bring the experience and empathy that comes from raising a son on the autistic spectrum with ADHD, who graduated university with a 2.1 in Mathematics and now works here in Cayman for a Big 4 firm. I am passionate about helping neurodiverse children thrive, through the right diagnosis, care and support, and that includes advising and supporting parents because neurodiversity can come with a complex set of challenges at a family level too, not just school.
Are you the best children's doctor in Cayman?
We are blessed with many fine paediatricians here and I’d encourage you to be wary of anyone who says they are the best, because nobody is the best at everything and they aren’t always the best for your specific child. It’s important to think about what you want or need and then find out which is the best paediatrician for you, by asking for recommendations or visiting them. You are always welcome to visit me prior to registering. It can also be worth looking at testimonials and things like Google Reviews, or asking in Facebook Groups. I am certainly proud of the ratings I get and the lovely things my parents say.
Do you offer telehealth or teleconsultations?
Yes, I do. And I was offering them before Covid too. Much depends on the nature of care required. Some things don’t lend themselves to terleconsultation and all good paediatricians value their direct interaction with the child, which helps for a more holistic health view. However, there are also many scenarios where it is better to have a teleconsult with your known paediatrician than finding a new one who does not know your child.
Can you help our child if we are travelling?
Absolutely. Cayman is a nation of travellers and many of my families travel for extended periods in the summer and at Christmas. I spend a fair bit of time providing advice and having teleconsults with my patients and parents. The one limitation is that paediatricians are generally tied to a jurisdiction for prescribing. So, if you child needs a prescription whilst away, that may necessity goign to a local GP or Paediatrician for that aspect.
Paediatrician or Pediatrician - what's the difference?
Well, the simplest answer is that there is no differences beyond the UK versus US spelling. However, that’s not 100% true. As a ‘Paediatrician Cayman’, we can come from either system and another 5 key jurisdictions too, so it is helpful to understand how the terms are used back home.

In the US, a pediatrician can be office based only, hospital-based for purely pediatrics, and can be specialised in neonatal medicine. It’s all lumped under ‘pediatrician’, meaning to find out someone’s real specialist training, you have to ask.

In the United Kingdom, we use Paediatrician for children and Neonatologist for newborn and neonatal problems. So, someone who trained in the UK would use paediatrician when their expertise is general or specialist paediatrics, and neonatologist if they held formal accreditation in neonatal medicine. I am both, with formal accreditation as a neonatologist.

Urgent Paediatric Care

Paediatric urgent care takes many forms, from simple advice to a an emergency hospital admission. I am extremely available for urgent paediatric care, and for the more serious issues, I have up-to-date paediatric and newborn resuscitation skills, and full admitting rights to the Cayman Islands Hospital.

Out-of-hours Access

I pride myself on my availability for urgent or out-of-hours paediatric care and ‘always available’ is one of the things my parents love most. I live permanently in Grand Cayman and only a short drive to the clinic. Whilst on island, I  am permanently on call for deliveries and neonatal patients, 24 hours per day, 7 days per week. However, you’ll also find me extremely accessible out of hours for paediatric urgent care. You can phone on 326-7827.

Hotel Visits

Are you on holiday but in urgent need of a paediatrician? I can provide urgent paediatric care for hotel and condo families, at the exact same charges as my local population (child holiday illness is stressful enough). It’s important to remember that nowhere is very far in Cayman and so often it is better to attend the clinic, where I have more extensive equipment, but this dependent on each case. Hospital Admissions

I carry admitting rights to Cayman Islands Hospital (HSA) in George Town (both for newborns and for the paediatric wards). I work closely with the HSA nursing teams, ER and ward staff to ensure efficient admission and coordinated care, whether it is simply for observation and monitoring or whether it is something more serious. If you feel your child may be sufficiently unwell that a hospital admission is warranted, in the first instance please call me on 326-7827. However, in ALL paediatric emergencies, please head straight to the Emergency Room at HSA, as it is better to set off and call me on the way.
